Welcome to Lighthouse Primary Care. You are welcomed as a new patient to our practice, and we are excited that you are partnering with us in your healthcare journey. To become a new patient, please complete the Online Intake Form. Once we receive your form, we will contact you with next steps. If you are transferring care from another provider, please complete the Medical Release Form.

Medical release forms are available to download here, or to pick up at our office. Please print the form, complete it, and drop it off at the office or fax it to 800-507-3166 if you are transferring care here.
If you transfer care to another provider from Lighthouse Primary Care, we have a copy service that copies the records here weekly. There is a fee to obtain records. Please call the office for pricing details.
